Covering 6 GP practices in Bolton, England, I see musculoskeletal patients on behalf of the GP (around 35% of all patients visiting primary care) to reduce the workload from my medical colleagues.
The patients are undiagnosed and undifferentiated so it is important to screen for masquerading symptoms and serious pathology. I can request Xrays, ultrasound scans and bloods to help accomplish this. I then manage these patients conservatively or refer to other services such as rheumatology, orthopaedics, pain management or physiotherapy for more conventional treatment.
Advanced Physiotherapy Practitioner

I worked as an APP in GP practices between Preston, Chorley and Euxton diagnosing a wide range of complex MSK conditions
I referred on to other local services such as physiotherapy, rheumatology, pain management or secondary care, always ensuring the process is done as quickly and efficiently as possible to save delay and reduce cost.
We had weekly meetings with spinal and upper limb consultants to discuss service changes, patient case studies and ways we can improve joined-up working. We also have an MSK meeting once per month to discuss any local policy and pathway changes and take it in turns within our team to deliver in service training.
Once per week I triage all the referrals to the MSK service. I prioritise them, return them with advice if the information is insufficient or refer them elsewhere if appropriate to reduce wasted appointments.

2018 - 2019
Working in a CATS service (Clinical Assessment and Treatment) along side nurses and orthopaedic consultants. We had an Xray and MRI scanner on site. Our role was to try and reduce surgical wait times and increase the conversion rate. For patients on the surgical waiting list - we ensured they had the appropriate investigations, correct outcome measures and had tried all the conservative options first.

Since qualifying, I worked in the NHS as a physiotherapist in a musculoskeletal outpatient department. we received referrals from the GP as well as self referrals. I treated my own caseload of patients through exercise therapy, hands on treatment or referring them to other services such as occupational therapy or pain management clinic for further input.

Additionally:
I first qualified as a physiotherapist in 2012 and since then I have working in musculoskeletal outpatients in the community treating a wide range of conditions.
Since February 2021 I have worked as an FCP/musculoskeletal specialist covering 6 GP practices. I diagnose a wide range of complex MSK conditions autonomously and quickly using clinical skills and experience as well as image investigations and blood investigations. Once I have the accurate diagnosis I discuss the appropriate options and share the decision making with the patient.
I refer on to other services such as physiotherapy, rheumatology, pain management or secondary care, always ensuring the process is done as quickly and efficiently as possible to save delay and reduce cost.
We have an MSK meeting once per month to discuss any local policy and pathway changes and take it in turns within our team to deliver in service training.
I participate in providing training to our band 7 colleagues and I am the Radiology lead for our team.
I have previous worked as a guest lecturer at Manchester Metropolitan University for students on the first-contact practice Master’s module.
